“Connected – display OFF” message shows up on ipad mini 2,
on selecting Duplicate Monitor in Windows (“Windows key + P” >> Duplicate).
I’ve attached zip log file for your reference.
I also often get the following error after a sudden and abrupt disconnection:
“Disconnected
Disconnected by server error
Error code: 2-5-6-5-6-

Please check if your primary graphics adapter is updated and has WDDM version 2.0-above.
You can check the WDDM version by opening the dxdiag.exe then click Display tab, check the “Driver Model” underneath “Drivers” box.
